Application checklist
Please take the following steps to help us process your application quickly:
- Check that the form has been completed correctly.
- Check that the form is signed and dated.
- Ensure that any additional sheets are clearly labelled and attached securely to the form.
- Ensure that the cheque is signed, dated and attached securely to the form.
- Confirm that any relevant documentation is enclosed, including copies of proof of change of name, for example a deed poll or marriage certificates.

Form RF1 only
Please check whether any solicitors are subject to section 12(1) of the Solicitors Act 1974. If we are aware of solicitors subject to section 12(1) when we pre-print your form, their details are included in Section G; an additional form (RFs12) must be completed before their practising certificate(s) can be issued. This also applies to registered European lawyers (RELs) who are subject to the categories listed in Section G, 7.3.
Form RF3 only
Please do not send RFs12 forms or accountants reports with this form. They must be submitted separately.
