The TOC defaults to operating only on headings contained within the same

An allowed header element may be limited to only those elements carrying
a specific class by setting the element to element.classname (so 
"span.incl" will allow <span class="foo incl bar"> but not 
<span class="foo bar">). You cannot currently require more than one
simultaneous class (so span.foo.bar will not work).
