Partnerships

  • One partner in a two-partner practice has died or has left unexpectedly, or
  • A partnership has split leaving two or more partnerships, sole practitioners or a combination.

You need to complete a FA5 - Application for Temporary Emergency Authorisation application.

Save the form before you start completing it.

The application form is an editable PDF document and needs to be filled in electronically.
