- One partner in a two-partner practice has died or has left unexpectedly, or
- A partnership has split leaving two or more partnerships, sole practitioners or a combination.

How to apply
You need to complete a FA5 - Application for Temporary Emergency Authorisation application.
Save the form before you start completing it.
The application form is an editable PDF document and needs to be filled in electronically.
Send your application to the email address on the form. Please include "temporary emergency authorisation" in the subject line.
There is no fee for this application.
What you can expect from us
We will acknowledge and allocate your application within one working day.
We aim to make a decision about your application within seven working days.
