Use this box to detail any further information. Examples include
- a change in your position in your firm or organisation;
- if you have decided not to renew your practising certificate, the reason for your decision;
- the date from which you wish your practising certificate to be effective, if other than 1 November 2008; and
- a change to your name and the date it was made.
